Have you considered your end-of-life wishes? Have you expressed those to your family, loved ones, and caregivers?

Advance Directives are legal documents that allow you to outline your decisions about end-of-life care. They give family and healthcare providers guidelines for how to best support you at the end of your life. We will explore the importance of such documents and offer tools to create your own.
